As a Wound Care Certified Nurse Practitioner, you will oversee care for members with complex and chronic wounds from initial assessment through closure or transition of care. You will review high-definition wound images and clinical data through a secure, AI-enabled platform, independently apply clinical judgment, and address broader health and social needs affecting healing. As Clover expands its Home Health program, you will also support clinical review, care coordination, and program development.
