Starting January 1, 2027, the current PlusTeam will become part of the Municipality of Geldrop-Mierlo and continue as Team Plus Social Domain. From this new position, we will continue collaborating with the Municipality of Waalre and supporting residents of both municipalities. Jack van der Kruijs - Transition Manager Your Challenge You provide process management for complex, multi-faceted, and gridlocked cases, creating cohesion among involved parties in the care, criminal justice, and municipal domains. You investigate what is going on, map out the request for help and self-reliance, and draw up a support plan together with residents, their network, and professionals. You coordinate specialized support, monitor progress and results, and intervene where necessary. You act decisively in (impending) crisis situations, prevent escalation, and make difficult decisions when needed.
