Organization: TX Organ Proc and Presv Job Summary: Facilitates and coordinates the evaluation of potential transplant and/or living donor candidates. Assists with coordination of care and discharge process for recipients hospitalized for transplantation and living donation and post-operative complications. Provides clinical management transplant recipients and/or living donors, who are followed as outpatients, in collaboration with the transplant team (attending physicians, nurse practitioners, clinical transplant pharmacists, social workers). .
