POSITION SUMMARY The RN Clinical Consultant is a key member of Titanium Healthcare ’s Enhanced Care Management (ECM) program, providing clinical oversight and guidance to support high-quality, coordinated care for complex member populations. ECM is a benefit under California Advancing and Innovating Medi-Cal (CalAIM), designed to deliver comprehensive, person-centered care that addresses medical, behavioral, and social determinants of health. In this role, the RN Clinical Consultant partners closely with Lead Care Managers (LCMs) and interdisciplinary team members to ensure care delivery is clinically appropriate, compliant, and aligned with healthcare plan requirements. The Clinical Consultant supports clinical decision-making, risk stratification, and care planning across the ECM population. The RN Clinical Consultant is responsible for oversight of Comprehensive Assessments, Care Management Plans, Transitions of Care, and clinical escalations, ensuring timely interventions and continuity of care.
