Population Health Care Manager - Registered Nurse or LPN

Must be Registered Nurse or LPN in state of Illinois Location: State of Illinois Shift: 7:30a-4p, MON-FRI Time Type: Full-Time Facility/Clinic Name: Fully Remote Position Summary Responsible for serving as a patient advocate. Enhances a collaborative relationship with the patient, family and clinician. Serves as a health coach to promote healthy lifestyle for patients within cohort. Partner with stakeholders to improve quality outcomes and measures. Principal Accountabilities Standards of Performance: Respect, Integrity, Compassion, Collaboration, Stewardship, Accountability, Quality Education Associates degree in nursing required. Licenses and Certification Current LPN nursing license, pursuing RN; RN strongly preferred Experience and Skills Technical: Minimum of three-year experience in the use of Electronic Medical Record systems, EPIC preferred. Strong background in ambulatory and/or inpatient acute care nursing Ability to effectively communicate with physicians, care team members, and caregivers to maximize individual effectiveness. Clinical acumen to support and understand disease progression and common associated interventions Basic knowledge and experience using web-based meetings and communications, Excel, Word, PowerPoint, and other software applications.
