In this fully remote role, you will support patients enrolled in Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) programs by providing ongoing education, care coordination, and clinical support. Working collaboratively with physicians, pharmacists, nurse practitioners, and other healthcare professionals, you will help patients better manage chronic conditions, improve medication adherence, reduce unnecessary healthcare utilization, and achieve better health outcomes. Key Responsibilities Conduct comprehensive patient assessments to identify clinical, educational, and psychosocial needs. Provide patient education in both Spanish and English regarding chronic disease management, medications, nutrition, preventive care, lifestyle modifications, and self-management strategies. Develop and update individualized care plans in collaboration with the interdisciplinary care team.
