The Role We're looking for a Licensed Practical Nurse (LPN) - Virtual to provide remote clinical support, telephonic triage, and care coordination. In this 100% remote role, you will interact with patients, families, and healthcare providers via phone, video, and electronic messaging to monitor treatment plans, coordinate care, and ensure clinical compliance. What You'll Do Conduct telephonic and virtual clinical intakes, health screenings, and wellness checks with patients and caregivers. Perform telephonic triage for symptom-based calls using established protocols and algorithms, directing patients to appropriate levels of care. Serve as a primary virtual point of contact for patients, families, and interdisciplinary care teams. Review and update individual plans of care under the supervision of Registered Nurses (RNs) or physicians. Track remote patient monitoring (RPM) data, vital sign trends, and clinical records to flag gaps in service or changes in status.
