Principal Care Management (PCM) Nurse

This is a remote position. Only LATAM-based candidates (bilingual in Spanish and English) Location: Latin America (Remote) Schedule: Full-time – U.S. business hours (EST) Payment: $1,200 USD/month (depending on experience) We are seeking a licensed nurse or MD to support a U.S.-based endocrinology practice through its Principal Care Management (PCM) program . PCM is a care coordination program designed for patients with a single chronic condition requiring ongoing management . In this role, you will work closely with the provider to ensure patients receive continuous support between visits, helping improve outcomes and adherence to treatment plans. This is a clinical, patient-facing role , requiring strong judgment, communication skills, and the ability to actively monitor and guide patients throughout their care journey. Key Responsibilities Maintain ongoing communication with patients enrolled in the PCM program Monitor patient conditions, including: Symptoms Medication adherence Progress and response to treatment Provide patient education regarding their condition and care plan Assist in the implementation and follow-up of care plans Coordinate services and ensure continuity of care Identify changes in patient status and escalate concerns to the provider when needed Ensure patients follow treatment recommendations and scheduled appointments Accurately document all patient interactions and updates in the system Requirements Licensed Nurse or MD (required) Advanced English l
