Clinical Partnership Manager

Mindoula ’s teams of case and care managers, supported by technology including a mobile engagement app, proprietary psychometrics, and predictive analytics, are the key to its success. Mindoula expects to expand rapidly across multiple business lines in multiple states and is seeking an experienced professional for its Clinical Program Manager role within the Collaborative Care Program. Mindoula ’s Collaborative Care Program (CCP) is a virtual behavioral health overlay based on the University of Washington-based AIMS Center’s Collaborative Care Model. CCP works in conjunction with primary care, women’s health, and specialty physician groups to provide care to patients with mild-to-moderate behavioral health needs. It is estimated that upwards of 20% of patients in these groups may be experiencing mental health/behavioral health distress. Mindoula signs service contracts with groups, whose providers then refer select patients to CCP while continuing to play an ongoing role in treatment. The Clinical Partnership Manager (CPM) will grow to be a Collaborative Care expert focused on driving, building, and nurturing current and new partnerships within our Collaborative Care Program. This work will occur at the account, practice, and provider level with the ultimate goal of achieving specific referral volume goals and increasing referral volume. Ultimately, the Clinical Partnership Manager’s main job is activating physicians within contracted practices to refer patients to the CCP s
