Category: Education Location: HNO6005- Complex Care Coordination As an experienced online educator, the candidate will work as an independent contractor of LINC Education to offer online facilitation services for students studying in theMasters of Nursing (Online) offered by Victoria University Online. Candidate must possess strong knowledge of complex care coordination and person-centred care, with the ability to apply evidence-based frameworks, evaluate models of care, and navigate legal, ethical, and cultural considerations. Proficiency in collaborating across services, managing chronic conditions, and utilising technology for care planning is essential. Key Skills Required Safety awareness Care Plan Management Care giving, empathy, and compassion First Aid Mobility and dexterity assistance Therapy and rehabilitation Patient-centred care Care transitions Desired Candidate Profile Registered Nurse or Midwife (AHPRA registration) or Registered Mental Health Practising Nurse and knowledge of unit content.
