The Opportunity: We are seeking a highly organized and detail-oriented Quality Nurse Analyst. This position is remote and offers you the flexibility of working from home. Essential Role and Responsibilities: Review clinical incidents. Follow standard operating procedures (SOP) to determine root cause, risk, and resolution for incident. Process interim medical orders as needed to notify physician of patient needs and changes in condition such as fall, wound, injury and post-hospitalization Perform follow-up calls for patients recently discharged from acute hospitalization and who are considered high risk for readmission Communicate as needed to client, caregiver, coordination teams, payers (case management / service coordination), field RNs, providers and key stakeholders to ensure client safety and continuity of care. Follow regulatory guidelines to manage clinical incidents to key deadlines. Share trends and insights noted in aggregate data to support continuous quality improvement.
