Position Summary The Nurse Care Coordinator (RN) is an integral member of Clarity Pediatrics ' interdisciplinary care team, collaborating with physicians, advanced practice providers, psychologists, and therapists. Under the direction of clinical leadership, the Nurse Care Coordinator ensures seamless care delivery for children with chronic conditions by coordinating clinical appointments, providing patient and family education, managing medication authorizations, and serving as a primary point of contact between families, providers, and pharmacies.
